There are 2 forms that need to be filled and handed in on Friday.
You guys should already have 1 of it (the following picture),so fill this in if you have it.
If you've lost it,bring along the following information on Friday to complete this form:
1.Birth cert. number
2.Place of birth
3.Date you entered this school
4.Name and address of previous school.

The second form is:


This form will be given to those that haven't got it on Friday.This form is easier to fill in,so you won't need to bring any special information for it.
